- A current referral from your GP or specialist
- Your Medicare card
- A list of your medications, including doses
- Recent test results, letters, or hospital discharge summaries if you have them
- Your pacemaker or defibrillator identification card, if you have a device

Do I need a referral?
Yes. Please bring a current referral from your GP or specialist. This is required for a Medicare rebate on your consultation.
How do I make an appointment?
Appointments are made by telephone. Call our reception team on 02 6622 3088, Monday to Friday, 8:30am to 5:00pm.
Which rooms should I attend?
We consult in Lismore, Ballina and Maclean. Reception will confirm the location when you book, according to the doctor and the tests you need.
Can my GP refer electronically?
Yes. We accept electronic referrals through Specialists & Referrals, as well as fax to 02 6622 3081 and paper referrals brought to the appointment.
Do you see patients in hospital?
Yes. Our doctors visit Lismore Base Hospital and John Flynn Private Hospital, providing both inpatient and outpatient care.
